Safety program and W-9: The written safety program, JHA/JSA and rescue procedures, and a W-9 issued in the legal name that matches the certificate of insurance.
State contractor licences: General contractor licences for the states where HexTel self-performs. Licence numbers and status released on request.
Certificates of insurance: GL, auto, workers compensation and umbrella. COI with additional-insured endorsements issued per program requirement through our broker.
Bonding: Surety letter stating single and aggregate capacity, issued on request for bonded scopes.
NAICS, SAM.gov UEI and CAGE: Company identifiers for federal and prime-contractor files, released to procurement on request.
Crew certifications: NWSA, ComTrain, OSHA 10/30, RF awareness and first aid/CPR by headcount, plus the tower program alignment to ANSI/ASSP A10.48 and TIA-222.
Named references: Carrier and prime program references released under NDA with a title and company.
